Social Work and Discharge Team
The small team of social workers and discharge co-ordinators are employed by Papworth Hospital NHS Foundation Trust.
The Social Work and Discharge Team arrange either rehabilitative care or social care on discharge for patients who require support with personal care on discharge.
The social workers also provide advice on a wide range of social issues for patients and their families at the hospital.
Department Work times:
Monday to Friday 09.00 to 16:30. The office is closed at weekends and Public holidays.
